Can anyone tell me the make and part # of the proximity switches used on the PRT.

Pricing here is for the pair of X/Y switches and cable, but single proximity switches for special uses are also available. Bracketing, targets and instructions are included in the kit.

I would call Shopbot to make sure these will fit a PRT

I needed the manufacture and # to find out the spec's. I'm converting a prt to Gecko drives and a Bob Campbell breakout board. The board uses 12 volt for their prox switches and shopbot manual shows 5 volts. Most prox switches have a wide range on the voltage and I just want to make sure 12 volts is within spec's.

I am fairly sure that they are 24V NO NPN configuration with the prox for the X & Y in parallel.

12-24VDC +/- 10%
However they seem to work at 5VDC!

When one of my sensors started to become flaky, I replaced the Shopbot supplied proximity sensors with the APS4-12N-E-D sensors from Automation Direct. Those sensors are slightly larger. They work perfectly with 12VDC and they cost $16.75 each.

If you decide to do the same, you'll need to make your own brackets. I made some out of 1/8th-inch aluminum. Total cost for two sensors, two brackets, and two quick-disconnect fittings (so that I could easily replace a sensor without using a soldering iron) was less than $50.
