I was thinking about a vinyl sign for my car and wondered if you could print on part of the vinyl (say black letters on white bkgd) then cut out part of the design with a vinyl cutter like from widgetworks to make a 3d design?

Yes you can.

It is done with sign industry printers that have outdoor printing inks that print on different types of sign vinyls and materials.

Some have built in cutters with optical recognition so you can remove the print and laminate it,(recommended) and then put it back on the machine and cut it.

You can also flood your substrate with vinyl and rout through just the vinyl to expose the substrate.

They also have substrates called color core where the face is one color and the core is a different color. You rout through the face to expose the core color

the company that I do work for has a high tech printer that will print on most any substrate. how wide and long do you need it they can print 12 foot wide and as long as the material is. they printed some banners that were 150 feet long but they can print longer.
